Take Next Steps
Some of the next steps may include:
- Admission for a time-limited trial of aggressive therapy
- Changing the approach to more aggressive Symptom Management
- Admission for management by the Palliative care service
- Discharge Home with referral to home-based palliative care service
- Discharge back to long-term care for a comfort measures approach

very important
Don’t forget to ask the family if they are able to cope at home if taking patient home. Families can be made to feel guilty if they are not prepared to care for someone dying at home. This is a very stressful situation and all family members have to be on board.
ok
We are often limited by resources in the community as we are in a rural setting.
we are limited by lack of resources in the community. Sadly admission occurs more frequently than it should
admission can have risks but has benefits

All good suggestions, obviously case-by case dependant upon the patient and family’s goals and needs.
Great suggestions.
All great suggestions. I would add some family support (counselling etc ) especially if this a new issue for them.
which ever is suited for the family and their needs, although based on previous conversations the first 2 don’t seem to be inline with the family goals
All of these suggestions are good. The only one I could add is admission to a hospice setting where his symptoms could be managed and it is a home type environment where the family can visit and spend time with the person.
all the suggestions are entirely valid and have used almost all of them at one time or the other.
