We have a community paramedicine program we can refer high risk and frequent callers to which can help to connect community resources to the patient. This can be especially beneficial for seniors who are having issues with falls.

fall screening on all ER patients, we also have a community paramedic program that we refer high risk patients to so that they can have in home visits to prevent falls and allow patients to stay at home rather than feel like a prisoner in their own home.
